Wellness 360 is a comprehensive case management service MicroHealth helped design and implement supporting UMED to help individuals achieve wellness which we define as a state of complete physical, mental and social well-being, not merely the absence of disease or injury. Clients who require case management are typically complex in nature. Current case management practices are often problem-oriented not person oriented treating a symptom not necessary the individual. Unless you address the multitude of factors contributing to the individual’s well-being, solving one dimension of an issue won’t necessarily result in a state of wellness. Rather than take a problem-oriented approach typical of case management services, we take a 360-degree look at the individual, not just the problem to help that client achieve wellness. Clients may have a constellation of needs that include but not limited to Medical, Dental, Behavioral, Education, Employment, Family, Housing, Legal, Nutrition, Spiritual or Support network. Our 360-approach begins by performing a comprehensive assessment on referred client’s needs. We then formulate goals to help meet those client’s needs. Based on the agreed upon goals with the client, we then formulate a plan of action that includes facilitation, care coordination, evaluation, and advocacy to help our Clients achieve these goals and a state of wellness. The result is a comprehensive set of services that take advantage of available resources to promote quality, cost-effective outcomes and most importantly, wellness for the Client.
